How do I apply for a True & Attested (official) Public Driving Record?

You may request a True & Attested Public Driving Record Online, by mail, by phone, or in person only at the Haymarket/Boston Registry of Motor Vehicles (RMV) Branch.

You will need to complete the Public Driving Record Request form

  • In Person: Submit a completed Public Driving Record Request form and $20.00 fee. Cash, check or money order, made payable to MassDOT.
  • By Mail: Submit a completed Public Driving Record Request form and mail with a $20.00 check or money order, made payable to MassDOT (Do Not Mail Cash).
  • By Phone: You can request a Public Driving Record by calling the Contact Center Mon-Fri from 9:00am to 5:00pm.

When will I receive my True & Attested Public Driving Record?

  • You will receive your True & Attested Public Driving Record within ten (10) business days when processed online, by mail, or by phone.
  • You will receive your True & Attested Public Driving Record in hand when processed in person at the Haymarket/Boston RMV Service Center.

How do I apply for an Unattested (unofficial) driving record?

You may request an Unattested Public Driving Record Online, by mail, or in person at any RMV Service Center.

You will need to complete the Public Driving Record Request form

  • In Person: Submit a completed Public Driving Record Request form and $20.00 fee. Cash, check or money order, made payable to MassDOT.
  • By Mail: Submit a completed Public Driving Record Request form and mail with a $20.00 check or money order, made payable to MassDOT (Do Not Mail Cash).

What is the fee for an Unattested Public Driving Record?

  • $6.00 when processed through our Online Transaction
  • $20.00 when processed via the mail, phone or in person.

Please note: If you currently reside out-of-state, indicate in your request where you would like your driving record mailed.

When will I receive my Unattested Public Driving Record?

  • You will receive your Unattested Public Driving Record at the end of your online transaction. It will be in PDF format and requires Adobe Acrobat Reader which can be downloaded for free from Adobe.com.
  • You will receive your Unattested Public Driving Record within ten (10) business days when processed via the mail, or by phone.
  • You will receive your Unattested Public Driving Record in hand when processed in person at a RMV service center.

How do I apply for a Personal Driving History or Full Certified Driving History?

You may request a Personal Driving History or Full Certified Driving History by mail or in person only at the Haymarket/Boston RMV Service Center.

You will need to complete the Authorized Release of Personal Driving History/Full Certified History form

In Person: Submit a completed Authorized Release for Personal Driving History/Full Certified Driving History form and $20.00 fee. Cash, check or money order, made payable to MassDOT

By Mail: Submit a completed Authorized Release for Personal Driving History/Full Certified Driving History form and $20.00 check or money order, made payable to MassDOT (Do Not Mail Cash).

The fee for a Personal Driving History or Full Certified Driving History is $20.00

When will I receive the Driving History?

  • You will receive your Personal Driving History within ten (10) business days when processed via the mail.
  • You will receive your Personal Driving History in hand when processed in person the Haymarket/Boston RMV Service Center.

When will I receive the Full Certified Driving History?

  • You will receive your Full Certified Driving History within ten (10) business days when processed via the mail.
  • Your Full Certified Driving History will be available for pick up 24-48 hours after the request is made in person at the Haymarket/Boston RMV Service Center. You may also elect to have the response sent via the mail.
