Mass ID or Liquor ID Card Application

All questions in the Required Information section and the Voter Registration section must be answered regardless of the type of transaction.

If you are under 18 years of age, the Parental Consent For Minor section must be completed by a parent/guardian for Mass ID applicants.

Bring the completed form to any full service RMV branch office with the appropriate fee.
School Pupil Transport (7-D) License Application Complete and sign the form in the appropriate places. Make sure your physician completes his/her section on the bottom of the form.  Mail the application and correct fee to the address on the form.

Renewal of a License/ID Card Applications

Road Test Applications

Road Test Application for a Commercial Driver's License Complete the General Information section, the Required Information section, the Sponsor Information section, the Vehicle Requirements section, and the Signature section.
Road Test Application for Military Skills Test Waiver The first page of this form must be completed and signed by the service member requesting the skills test waiver based on their military driving experience. The second page of the form must be completed and signed by the service member’s Commanding Officer. The Commanding Officer’s determination for the class of vehicle to select should be based on the a review of the service member’s military training records, the US Government Operator’s ID/License, and the military driving record.

The completed form should be mailed to MassDOT, RMV Division, Driver Licensing, PO Box 55889, Boston, MA 02205, or faxed to Driver Licensing department at 857-368-0820 for review. You will be notified of the result of your request either by phone or email notification.

CDL Self-Certification Form

Form Name Instructions
CDL Self-Certification Form Complete the Commercial Driver Information section of the form, select a self-certification category, and sign and date the form. If you are required to carry a medical certificate, you must fill out the Medical Examiner’s Certificate Information section found on the second page of the form, and will need to provide the RMV with a copy of your medical certificate. If your medical examiner’s certificate is only valid with a vision, diabetes, or skills performance evaluation variance granted by the Federal Motor Carrier Safety Administration (FMCSA), you will also need to provide this documentation to the RMV when you submit your CDL Self-Certification Form.

Class D and M Vision Screening Certificate Have your eye care provider complete and sign this form in the appropriate sections. Bring the completed Vision Screening Certificate with you when you come in to apply for or to renew your license.

Intrastate Medical Waivers to Operate Class A, B, or C Commercial Motor Vehicles Application A copy of the results of a recent DOT medical examination performed pursuant to 49 CFR 391.43, upon which the examining physician has indicated that you are only qualified to operate a commercial motor vehicle with an intrastate medical waiver. Your employment driving record for the previous five years. Your current employer's letter(s) of recommendation, including a certification that you will be engaged only in intrastate commerce within the Commonwealth of Massachusetts.
Diabetes Medical Evaluation Form for School Pupil Transport (7D) The form must be completed in its entirety and signed by a medical doctor that is board-certified or board-eligible in endocrinology. In addition, the applicant must agree to adhere to the conditions outlined in the form, and sign the form in the presence of the endocrinologist. The form must then be submitted in conjunction with a completed 7D-School Pupil Transport License Application.
Cardiovascular Medical Evaluation Form for School Bus and School Pupil Transport (7D) Operator The form must be completed in its entirety and signed by a medical doctor. The form must then be submitted in conjunction with a completed 7-D School Pupil Transport Certificate Application.

Ignition Interlock Affidavit - Additional Licensed Resident This affidavit must be completed by any new licensed resident living in a household with a driver who is required to have an Ignition Interlock Device installed in their vehicle. Please complete this affidavit and mail to: Registry of Motor Vehicles, Driver Control Unit, Attention Interlock Program, PO Box 55896, Boston, MA 02205.
Ignition Interlock Affidavit - Other Licensed Resident This affidavit must be completed by each licensed resident living in a household with a driver who is required to have an Ignition Interlock Device installed in their vehicle. Please complete this affidavit and submit to a Hearings Officer at the Registry of Motor Vehicles with proof of installation of the Interlock Device and any other affidavits that are required.
Ignition Interlock Operator's Affidavit This affidavit must be completed by drivers who are required to have an Ignition Interlock Device installed in their vehicles. Please complete this affidavit and submit to a Hearings Officer in addition to a proof of installation of the Interlock Device and all other required affidavits.
Ignition Interlock Operator's Affidavit for Out-of-State Residents This affidavit is required by non-Massachusetts residents looking to reinstate their right to operate in Massachusetts after two or more Operating Under the Influence (OUI) convictions. Please complete this affidavit and submit with an acceptable proof of Out-of-State residency to: RMV Driver Control Unit, Attention Interlock Program, PO Box 55896, Boston, MA 02205.
Class D, M, or D/M License and ID Card Application
 Use this form to:

Affidavit of Sponsor in Support of a Person Aged 65 or Older Seeking a Massachusetts ID Card This Affidavit must be completed and signed in an RMV branch by a qualified sponsor who is confirming the identity of a Massachusetts ID Card applicant over the age of 65. The Affidavit must be signed in the presence of the RMV Branch Manager or his/her designee.
Out-of-State Motorcycle Rider Education Program Application to Convert Complete all required sections. Enclose a check or money order and mail to the address on the form.
Out-of-State Driver Education Certificate Application to Convert Mail a copy of your out-of-state certificate, and a check for the required $15.00 fee, to the RMV Professional Driving School Section, P.O. Box 55889, Boston, MA 02205-5889.
Driving Record Request Form Complete all required sections. Enclose a check or money order and mail to the address on the form.
