Form Description Instructions
Application for Additional Repair, Farm, Transporter, and Owner Contractor Plates This application is used to request additional Repair, Farm, Transporter, and Owner-Contractor plates. Complete the Application for Additional Repair, Farm, Transporter, and Owner Contractor Plates and mail it to the address on the top of the form. The state police may conduct a compliance visit and your additional plates will be issued within 6-8 weeks.
Application for Additional Dealer Plates This application is used to request additional Dealer plates. Complete the Application for Additional Dealer Plates and mail it to the address on the top of the form. The state police may conduct a compliance visit and your additional plates will be issued within 6-8 weeks.
Section 5 Plates Cancellation Form This application is used to cancel a Section 5 plate. Complete the Section 5 Plate Cancellation Form and bring it to an RMV Branch or mail it to the address on the top of the form.
Application for Compliance Decals

This form is used to request compliance decals from the Section 5 Division. This decal shows a vehicle is in compliance with state sales tax and title requirements. Complete the Application for Compliance Decals. Instructions for completing each section are found on page 3 of the form. Bring the completed application and all supporting documents to the Section Five Division at 25 Newport Avenue Ext, or mail everything to the address on the form.
Application for Dealer Reassignment of Title (DRT-1) This application is used by Licensed Massachusetts Dealers to order DRT-1 forms used to reassign a title or assign & authorize a loan payoff or assign & authorize a duplicate certificate of title. Complete the top section and sign it. Mail or fax the completed form. The RMV will mail the DRT-1 forms to the Dealer address.

This application is used to request a dealer registration.

Complete the Application for Dealer Registration. Bring the completed application and all supporting documents to the Section Five Division at 25 Newport Avenue Ext, or mail everything to the address on the form.

Application for Farm Registration
This application is used to request a farm registration. Complete the Application for Farm Registration. Bring the completed application and all supporting documents to the Section Five Division at 25 Newport Avenue Ext, or mail everything to the address on the form.
Application for Owner / Contractor Registration
This application is used to request an owner/contractor registration. Complete the Application for Owner/Contractor Registration. Bring the completed application and all supporting documents to the Section Five Division at 25 Newport Avenue Ext, or mail everything to the address on the form.
Application for
Repair Registration

This application is used to request a repair registration. Complete the Application for Repair Registration. Bring the completed application and all supporting documents to the Section Five Division at 25 Newport Avenue Ext, or mail everything to the address on the form.
Application for Replacement Section 5 Plates This application is used to request a replacement plate from the Section 5 Division. If you can wait for your permanent plate to be manufactured and mailed: Bring the completed Application, your original registration, police report (if applicable), and the appropriate fee to any full service RMV branch office. If you cannot wait for your permanent plate to be manufactured and mailed: Bring the completed Application, your original registration, police report (if applicable), and the appropriate fee to the Section Five Division in Quincy. If you are requesting a replacement of a damaged plate, you will receive further instructions once the request has been processed with the Section Five Department.
Application for Transporter Registration
This application is used to request a transporter registration. Complete the Application for Transporter Registration. Bring the completed application and all supporting documents to the Section Five Division at 25 Newport Avenue Ext, or mail everything to the address on the form.
Name/Address Change Request Form This application is to be used to request a name and/or address change for Section 5 registrations. Complete this application for Name/Address Changes. You may mail or fax the completed application and all supporting documents to the address on the form.
